By completing an application form and proceeding to the payment page, I hereby agree to the terms and conditions of use for this website.

I am aware that the Disclosure and Barring Service, Disclosure Scotland, and Access Northern Ireland are Executive Agencies responsible for conducting specific criminal record checks upon request. They issue certificates containing details of an individual’s criminal convictions, or state that they have none. I authorize First Job Services Ltd through Staff Vetting Limited to verify the information provided in my application and proceed with a criminal record check for the purpose of obtaining a Basic Disclosure through the relevant Executive Agency.

Furthermore, I grant permission to the organization requesting this check to scan and upload the completed certificate into the online Basic Disclosure application system to facilitate the recruitment decision-making process. I confirm that my consent is explicit, fully informed, and freely given for the aforementioned purposes.

By signing this declaration, I affirm that all the information I have provided in support of the application is complete and accurate. I am aware that providing false information for this purpose is a criminal offense.

Moreover, I have read and understood the DBS statement of fair processing. I hereby give consent to the Responsible Organization to receive an electronic notification once the application has been processed. This notification will indicate either ‘Certificate contains no information’ or ‘Please wait to view applicant certificate.
